How Do We Enroll in Hospice Care?

Calling our office at (618) 277-1800 is the first step in the enrollment process. We will contact your physician in order to receive medical information to determine if you qualify under Medicare Guidelines.


The patient and or the family has the right to choose which hospice provider they would like to use.

All Hospices follow the same rules and regulations and all are paid the same.

Each individual Hospice decides how to provide hospice care and how often and what is covered under the Hospice Plan of Care created for each individual patient. The hospice will discuss this with the patient and family prior to signing up the patient so families are comfortable with the choice they make. A patient has the right to transfer to another program as outlined in the Benefit Information discussed upon admission.

Family Hospice wants patients and families to make good decision based on good information. Don’t be afraid to ask.

TREE OF MEMORIES

In supporting our Tree of Memories program through the holiday season, we make ornaments available at our kiosk at St. Clair Square from November 30 through December 22.
