Family Hospice is a Medicare Certified not for profit hospice program that began providing services in 1991.We proudly serve patients and families in Madison, Clinton, Washington, Randolph, St. Clair, and Monroe Counties. We work with the patient’s primary physician, family members and the patient in planning for the patient’s care. Our Hospice Medical Director is available to assist in accessing prescriptions and equipment to expedite obtaining supplies needed to care for the patient.

    The hospice medical director is available to assist in providing pain and symptom management including working with pharmacies to secure medication in a timely manner.
  4. EXTENSIVE GRIEF SUPPORT
    Children, teens, families and individuals in Southwestern Illinois have access to grief support and education through Family Hospice’s Heartlinks Grief Center, in business for over 25 years.
  5. PHYSICAL THERAPY, OCCUPATIONAL THERAPY AND SPEECH THERAPY
    Family Hospice often uses physical therapy, occupational therapy and speech therapy to identify patient limitations and patient care with the goal of keeping patients active.
  6. HOSPICE AND PALLIATIVE CARE CERTIFICATION
    The executive director, patient care coordinator, social workers and medical director have all received hospice and palliative care certification.
  7. IMMEDIATE FOLLOW UP, UPON REFERRAL
    When Family Hospice receives a referral, a staff member contacts the patient’s family and primary physician within 1-HOUR to identify the patient’s needs, establish a time for the initial visit and to assist with discharge planning.
  8. COLLABORATION TO ENHANCE AND SUPPORT OUR PERFORMANCE
    Family Hospice collaborates with other healthcare providers to enhance and support the performance of the primary care givers, improving the patient’s level of care.
  9. ACCREDITED MUSIC THERAPY
    Family Hospice provides a credentialed Music Therapy Program offered to patients and family to assist in improving the quality of the individual’s life.
  10. LINUS BLANKETS OF SOUTHEASTERN ILLINOIS PROJECT
    Family Hospice and Heartlinks volunteers have adopted the Linus Blankets of Southwestern Illinois to collect and distribute blankets for children facing serious illness and emotional stress.
